From the North:

• Take I-75 South
• Take Crooks Road Exit
• Turn Right from the ramp
• Turn Right onto Square Lake Road
• Continue on Square Lake Road Until Dequindre Road
• Turn Left on Dequindre Road
• The hospital will be to your left in about 1/2 miles

• Take I-75 North
• Take Rochester Road Exit
• Turn Right at traffic light
• Continue on Rochester Road until Square Lake Road
• Turn Right on Square Lake Road
• Turn Left on Dequindre Road
• The hospital will be on your left in about 1/2 miles

• Take M-59 West
• Take the Dequindre Road Exit
• Turn left at first traffic light
• Turn right at second traffic light
• The hospital will be to your right in 1/2 miles

• Take M-59 East
• Take the Dequindre Road Exit
• Turn Left at traffic light (Dequindre Rd. South)
• The hospital will be to your right in about 1/2 miles.

Parking Structure

Family Practice Center

• The parking structure is located just to the south of the hospital
• There is plenty of space and your car will not burn in the hot summer nor will freeze in the cold winter
• The cost for parking is now completely free!  (Valet excluded)

• It is located on the opposite side of the street from the main Hospital
• Space is limited, but parking is free and there is a shuttle available from there to the main entrance

• Come in through the main entrance
• Turn right and head towards Area C
• After passing the Registration Desk it will be to your right but it will be before the Chapel and the Cashier
• Take the elevators up to the 5th floor
• Turn left into the hallway and we will be the first office to your right.  Suite number 523.

• Come in through the main entrance
• Turn left and head towards Area A
• Pass the Pharmacy and take the Area A elevators down to the "G" floor
• Follow the signs to the Endoscopy Unit
